While standard residential care provides a supportive environment for those navigating mental health concerns, MMIS includes more intensive and specialized services.

What Are Medically Managed Intensive Services?

Level 4 medically managed intensive inpatient services refer to a level of care in mental health and addiction treatment that involves close medical monitoring and intervention for individuals with severe mental health conditions. These services are often provided in an inpatient or residential setting and are designed for individuals who require a higher level of care than what can be provided in less intensive programs. 

Level 4 programs can be found in three different types of settings:

  • An acute general hospital
  • An acute psychiatric hospital
  • A psychiatric unit within an acute general hospital with acute care medical and nursing staff

​These services are typically reserved for individuals experiencing severe mental health crises, such as acute psychotic episodes, severe mood disorders, or significant risk of harm to themselves or others.

Goal of Intensive Psychiatric Care

This level of care aims to stabilize individuals, mitigate immediate risks, and address the acute symptoms of their mental health conditions. The comprehensive approach involves close medical monitoring, medication management, and therapeutic interventions to promote stabilization.

MMIS works towards developing a tailored and sustainable plan for ongoing care, often involving collaboration with outpatient providers, family members, and support systems. Overall, the goal is to empower individuals to transition to lower levels of care as they progress in their mental health journey.

Treatment Structure

Medically managed intensive services involve a carefully crafted blend of medical expertise, therapeutic interventions, and vigilant monitoring, all tailored to provide a high level of care for those navigating severe mental health conditions. Comprehensive Assessment

In medically managed intensive services (MMIS), a comprehensive assessment process is pivotal in shaping individualized treatment plans. At the time of admission to our facility, a thorough nursing assessment is conducted. This initial evaluation is done by one of our medical professionals and contributes vital information to the treatment database, enhancing the foundation for effective treatment planning. Physician involvement is a fundamental aspect of MMIS, with a mandate for physician admission approval. 

​Within 12 hours of being approved for admission, one of our qualified physicians conducts a comprehensive history and physical examination of the patient. This examination is crucial for establishing the patient’s medical suitability for Level 4 care, ensuring they can benefit from these services.

A comprehensive biopsychosocial assessment, initiated at the time of admission, is a key component of MMIS. This particular evaluation explores the patient’s Substance Use Disorder (SUD) history, treatment experiences, biomedical condition, emotional state, behavioral patterns, and cognitive functioning.

Individualized Treatment Plans

Following a comprehensive assessment, a tailored treatment plan is meticulously crafted by a medical professional at our facility. This curated plan is not only reflective of the patient’s personal goals but also incorporates problem formulation and articulation of short-term and measurable treatment objectives. This treatment plan is dynamic, evolving in response to the unique needs and progress of the patient. 

​These plans specifically address Substance Use Disorder (SUD) recovery and co-occurring Disorder (COD) treatment and take into account any concurrent medical or mental health issues. The plan will also address broader lifestyle aspects, including attitudes, values, and behaviors that may impact the patient’s well-being.

​Regular reviews of the plan occur at specified intervals by our professionals, as indicated in the treatment plan itself. This review aims to ensure that the treatment remains aligned with the patient’s evolving needs and facilitates adjustments to the objectives and activities as progress is made.

Medication Management

​The goal of medication management is to optimize the use of psychiatric medications to address and alleviate symptoms, stabilize the individual, and support their overall well-being. For patients with co-occurring medical conditions, the medication management plan may also address the integration of medications to manage physical health issues alongside psychiatric treatment.

​Medication management works alongside other therapeutic interventions, such as counseling, psychotherapy, and behavioral therapies. The coordination of these elements aims to provide a holistic and synergistic approach to treatment, addressing both the biological and psychosocial aspects of mental health.

​Medication management involves regular reviews of the individual’s response to medications. Clinicians document the effectiveness, side effects, and any adjustments made to the medication plan.

Therapy

In Medically managed intensive services, the therapeutic interventions offered are designed to address the complex biopsychosocial needs of individuals grappling with severe mental health challenges. Professional medical staff engage patients in daily programming, encompassing medical and nursing care, motivational activities, readiness for change initiatives, and skill-building sessions. 

Cognitive, behavioral, motivational, pharmacologic, and other evidence-based therapies are delivered individually and in groups, depending on the patient’s requirements.  

Higher Level of Treatment Than Residential Care

​Medically managed intensive services are categorized by a high level of intensity and are designed for individuals facing severe mental health issues. With the goal of stabilizing acute signs and symptoms, MMIS often involves close medical monitoring, intensive therapeutic interventions, and 24/7 supervision. While residential care can vary in intensity, it generally provides a more moderate level of care compared to MMIS. Residential care may be suitable for individuals with a range of mental health needs, including those requiring a structured environment, but not necessarily at the acute crisis level.

​A key feature of MMIS that separates it from traditional residential care is the emphasis on medical management, particularly medication management. These programs often involve close monitoring by medical professionals, regular psychiatric evaluations, and the adjustment of medications to address acute symptoms and stabilize the individual. While residential care may include medication management, it may not be as intensive as in MMIS.

Residential treatment typically offers a broader range of therapeutic interventions, including counseling, group therapy, and skill-building activities, while MMIS focuses on stabilizing the individual. Treatment plans in MMIs are often centered around medical interventions, including psychiatric medications, in conjunction with intensive therapeutic modalities to address immediate symptoms.
